The problem.

The doc builds up the post or "crown extension" as this is called and
at impression time the whole thing comes out in the impression.

HONESTLY now dentists, do you know about what I am speaking?

We used to have that! It was called, "The Wonderful World of
ThreadMate Pins."

Later we found out it cracks natural tooth structure and resulted in
root canal therapy!

Reinforcing rods in concrete work wonders.
I would expect that sticking a (metal) rod in there would do the same.

I assume the standard question/answer would apply: How much of the
tooth is left?

Broken off at the gumline or BELOW the gum line. The gum bleeds into
the root canal space. The Dunkin Donut technique solves the problem.

PermaFlo™ - Indication & Procedure
Endo "Donut" Technique
PermaFlo Flowable Composite is used with dentin bonding to extend root
on compromised tooth.



1: When root canal is being performed on a severely broken down tooth,
remove decay and overlying soft tissue, then achieve hemostasis with
ViscoStat or Astringedent solution and Dento-Infusor device.



2: Note above how jaws of rubber dam clamp are barely engaging tooth.
The "Donut" technique achieves extension of root when retention of
clamp would other-wise be difficult or impossible. If retention of
clamp is not a problem, simply seal out saliva using OraSeal Caulking.




3: Prepare dentin for bonding by etching, then applying PermaQuick
Primer followed with PermaQuick Bonding Resin. Maintain a clean field
by use of vacuum only while bonding if initial clamp retention is
impossible.



4: Place donut shaped layers of PermaFlo, one upon another. Cure each
layer before adding additional layers. Although composite could be
used, the viscosity and delivery of PermaFlo is ideal. Because it is a
resin, PermaFlo bonds to the cured PermaQuick Bonding Resin which is
bonded to the dentin.




5: Use of opaque white PermaFlo contrasts with dentin aiding in
removal of material which may lap past root. Access opening is sealed
with SpTemp temporary cement for easy access following initial
treatment of canal(s). Root canal is completed as usual. Seal rubber
dam with OraSeal Caulking as usual if any leakage of saliva occurs
after "Donut" extension is completed.



6: After completion of root canal, "Donut" build-up functions as a
matrix for composite core with direct post placement. (We recommend
the PermaFlo DC System for post luting and core) Inside of PermaFlo
donut is freshened with a diamond bur and treated with PermaFlo DC
Bonding Primers (at same time dentin is primed) to maximize adhesion
of composite core to PermaFlo material. Note "Posts" in Endo-Eze
"Procedures" for technique of post bonding and luting with the
PermaFlo DC bonding and luting system.
